Treatment planning:

We will supply printed information on your strategy once your treatment strategy has been agreed with the Dentist. We will advise you and discuss this with you whether this strategy shifts due to clinical or radiographic findings. Treatment plans are not invalid for net 90 days from the date. Please do not hesitate to ask us, before the appointments for actual treatments are made, if there are any points on your treatment strategy that you want to query.

Consent forms:

Particular treatments necessitate completion of a written authorization form. This is in order to describe the treatment, aftercare and any risk to you personally thoroughly, before any of these treatments are completed.

Fees:

A credit account system will not manage and we require fees to be settled at the appointment.
Where treatment incurs a lab fee, at least 50% of the entire fee is due at the appointment where impressions are taken. Fees for specific treatments like Dental Implants and Invisalign are taken at each visit in staged payments.

Where treatment is paid for on finance through a Finance company, please take note that the finance arrangement must be accepted and signed before the beginning of that treatment. Please note a minimal deposit can also be required, and that finance is only accessible on treatments over?400, not including Emergency Dental Treatment.

Pearl Dental Clinic doesn't accept any payments.

Fees for treatment where intravenous sedation is comprised, must be paid prior to the appointment to prevent monetary transactions, or signatures being required, whilst a customer remains under the effect of the sedative.

Dream Smile Dental Clinic reserves the right to charge time based deposits for reserving appointments that are future. Deposits are deductible off the price of treatment.

All the fees employed for any kind of Dental treatment at the practice are based on; the stuff used, lab costs incurred (if appropriate), and the clinical time spent. The fees are nevertheless, illustrated as treatments prescribed on the treatment strategy for your information.

Late cancellation or missed appointments:
Dream Smile Dental Clinic reserves the right to bill up to?100 per hour, in the function of a missed appointment or an appointment.

We realize that some patients travel long distances to get to the practice, and in some cases being late for appointments can be inescapable. Please take note that you might be requested to reschedule your appointment if you're over 10 minutes.

Guarantee:

The patient has attended and regular assessment every 6 months.
Some treatments may have a guarantee of less than 1 year, and in this case you'll be informed by your Dentist either verbally or in writing, or both.

Private Details:

It is very important that you give the full medical history and details of any medication you take. Should these change in any way, it is essential for you to tell your Dentist. It is the patient's duty to inform the practice of any changes in either their medical history private details and/or.

Use of Pictures and X rays:

Dream Smile Dental Clinic may use pictures and x-rays of teeth and your smile only, for educational and marketing purposes on the Dream Smile Dental Clinic site, and on educational and promotional literature. Your name will never be printed, and identity WOn't be revealed. However, should you NOT want for us to use your pictures and x-rays in this way, please let's know.

Use of patient contact details:

We like to remind our patients of their appointments, when they're due for appointments, and other various reminders that are significant. On this particular note, you may be periodically contacted by the practice via mobile, text, e-mail or by letter in the post. Please inform US should you NOT want to be contacted by the practice by any or all these means.

Complaint's policy:

To be able to ensure that each and every patient has only the very best experience at all times at Dream Smile Dental Clinic we consistently take complaints about any aspects of our services very seriously.



Every complaint will receive immediate written/e-mail acknowledgment, and the Management will strive to resolve the complaint within an instant, reasonable time period (generally about 2 weeks). For our complete complaints process and policy, please request an associate of our reception team. It'll normally be acknowledged by the Management team and after that referred to the treating Dentist if a complaint is about any facet of clinical care or related charges.

If unsatisfied, you may want to forward your complaint to the General Dental Council.

No forbearance/Maltreatment policy

At Dream Smile Dental Clinic we manage a zero tolerance policy to mistreat to our Dentists and staff, loud/disorderly/drunken behavior, constant missing and late cancellation of appointments (after multiple warnings). In these situations, Dream Smile Dental Clinic reserves the right to deny treatment and entry.

Data Protection Act:

We save all patient personal details on a protected computer system in accordance with the Data Protection Act. All clinical notes, digital radio-graphs pictures etc that are digital, remain the property of Dream Smile Dental Clinic. Copies of radiographs, notes and pictures can be made accessible and Dream Smile Dental Clinic reserves the right to charge an administration fee for these.
